Additional Information: The complications associated with diabetes which can be macrovascular or microvascular triggered by the longstanding condition of hyperglycemia was studied. One of the major hyperglycemia mediated mechanisms that in turn mediate micro- and macrovascular complications is the formation of advanced glycation end products (AGEs). AGEs can be formed extracellularly, intracellularly and can be imported from food and tobacco smoke. They exert an adverse effect on health independent of hyperglycemia. Several studies have evidenced the role of AGEs in macrovascular complications like atherosclerosis and other cardiovascular diseases and in microvascular complications like nephropathy, neuropathy, and retinopathy. The macrovacular complicitions have been studied extensively while the microvascular complications though unique to diabetes are yet to be understood fully. Various animal experiments revealed that this glomerular hypertrophy is associated with an over-expression of type IV collagen, laminin B1, TGF-β and platelet derived growth factor. TGF-β is a prosclerotic cytokine which stimulates the synthesis of collagen matrix components such as fibronectin, laminin and protoglycans. Hyperglycemia and AGEs have shown to increase TGF-β concentrations. Due to accumulation of excess of type IV collagen and laminin and reduced amount of heperan sulphate in the glomerular basement membrane becomes more porous to macromolecules leading to albuminuria. The level of circulating AGEs in the diabetic patient are now being investigated to know their relation with the severity of the disease and hence have the potential to be developed as a marker for diabetes. AGEs are also important because their remediation have shown to improve diabetic micro- and macrovascular diseases. AGE toxicity could be averted by promising dietary and pharmacological strategies.
